CLIENT PRIVACY POLICY

 

Seaglass Wellbeing and Art Therapy collects and retains personal information about our clients for the purposes of providing psychotherapeutic art therapy services. Information is managed in accordance with legal and ethical guidelines of the profession. This policy outlines how Seaglass Wellbeing and Art Therapy maintains our client’s personal information.

 

Personal information we collect.

As a guideline, the following information will usually be collected:

 

  • Your name, address, and date of birth.

  • Your contact details.

  • Current and past medical information.

  • Current and past psychological history.

  • Your relevant family medical history.

  • Your educational and occupational history.

  • Legal or offending history.

  • Information obtained from psychological tests and questionnaires.

  • Information relevant to your response to treatment.

 

All the above material is used to establish an accurate assessment, and treatment plan for our clients.

 

Information provided by third parties.

Seaglass Wellbeing and Art Therapy may collect information from others who know you (e.g., a spouse or parent) or from others involved in your mental health care (e.g., your GP) with your consent. We endeavour to keep you informed about the information collected, and where appropriate, we will seek and obtain written informed consent from you to contact the third party to obtain that information.

 

Use and disclosure of personal information.

Seaglass Wellbeing and Art Therapy may be required by law to disclose personal information without the consent of the client. There may be occasions when Seaglass Wellbeing and Art Therapy discloses personal information of a client in the event of serious threat to the life, health, or safety of an individual or the public.

 

Storage and security of personal information

Seaglass Wellbeing and Art Therapy will protect the personal information of clients from misuse, access, or disclosure. Paper documents containing client information will be stored securely under lock. Electronic information containing client information will be stored and backed up securely in accordance with professional ethical requirements and professional best-practice.

 

Client access to your personal information

Clients have a right to access personal information held by Seaglass Wellbeing and Art Therapy. Access requests should be made in writing for security and recording purposes. Seaglass Wellbeing and Art Therapy may refuse the request or limit the information provided where legally entitled to do so. Refusal may occur when releasing information may have an impact on the privacy of others or is considered a risk to self or others.

 

Changes to this policy.

We may update this policy because of changes in legislation or based on professional best-practice guidelines. All clients are encouraged to review this policy to remain informed of privacy rights and obligations.
